Abstract: Abstract&#xD;
Leiomyomas or fibroids are the most common benign pelvic tumors in females that grow monoclonally from the smooth muscle cells of the uterus. Such tumors occur in nearly half of women over the age of 35 years, with increased prevalence during the reproductive phase due to hormone-stimulated growth. At 50 years of age, 80% of African and almost 70% of Caucasian women have fibroids. Following are 8 such case scenerios of fibroids uterus having different presentations, at RL Jalappa Hospital. The first 4 cases belong to the perimenopausal age group and the last 4 cases are the atypical fibroids presenting along with pregnancy. The various treatment modalities are opted as per the patient symptoms and situation. Uterine leiomyoma can present with various symptoms of which menstrual irregularities is most common. The management modality is purely based on the patient’s symptoms. Cesarean myomectomy decision is a challenge and will depend on the site and size of the myoma.</description>

      <title>Effect of oral nifedipine or combined with sildenafil citrate for management of threatened preterm labour randomized trial</title>

Abstract: Background: In developing countries, preterm birth is a leading cause of neonatal and infant illness and&#xD;
mortality. In southern India, there is an increase in preterm delivery and the prevalence is about 5-8%.&#xD;
It is related with severe suffering for both women and babies as well as long-term disabilities; hence&#xD;
interventions to prevent preterm birth are critical. The threatened preterm labor can be postponed by&#xD;
using “tocolytic” therapy in order to transfer the patient to a center with a neonatal intensive care unit&#xD;
or administer corticosteroids. It helps in the prevention or treatment of respiratory morbidity.&#xD;
Aims: To assess the efficacy of nifedipine combined with sildenafil citrate and nifedipine alone in&#xD;
threatened preterm labor and also to determine the maternal and perinatal outcome.&#xD;
Materials and Methods: The final analysis comprised a total of 60 subjects. There were 30 people in&#xD;
Group A (Nifedipine) and 30 people in Group B (Sildenafil +Nifedipine).&#xD;
Results: The mean age of the participants was identified as 21.07   3.31 years and 22.87   3.15 years&#xD;
in group A and B respectively. The cause of preterm was identified as idiopathic in the majority of the&#xD;
women in group A with 83.33 and in group B with 70% and 16.67%. Maternal side effects in group A&#xD;
were tachycardia, palpitations, nausea, and vomiting with 26.67%, 3.33%, and 16.67%, while in group&#xD;
B, tachycardia, facial flushing, palpitations, and headache were identified with 10%, 6.67%, 10%, 6.67%.&#xD;
Respiratory distress syndrome, Neonatal intensive care unit admission, perinatal death, alive and healthy&#xD;
neonates were identified with 16.67%, 23.33%, 20%, and 53.33% in group A, whereas it was identified with&#xD;
23.33%, 36.67%, 3.33%, and 56.67% respectively. The rate of success was higher with group B (93.3%)&#xD;
as compared in group A (50%).&#xD;
Conclusion: The combination of sildenafil citrate and nifedipine is more effective than nifedipine alone in&#xD;
avoiding approaching preterm labor.&#xD;

      <title>Association of Thrombocytosis and its Prognostic Significance in Cervical Cancer</title>

Abstract: Introduction: Thrombocytosis is associated with progression of &#xD;
many diseases. There is increasing evidence that tumour cells, &#xD;
platelets, endothelial cells interact with each other leading to &#xD;
spread of tumour cells into the microvasculature which results &#xD;
in poor prognosis due to metastasis. Thrombocytosis is an &#xD;
indirect marker of occult advanced disease.&#xD;
 Aim: To determine thrombocytosis in diagnosed carcinoma &#xD;
cervix patients and its association with stage of cancer cervix &#xD;
and prognosis.&#xD;
 Materials and Methods: This was the retrospective study &#xD;
conducted at Sri Devaraj Urs Medical College, Kolar, Karnataka, &#xD;
India, from April 2021 to May 2022, on medical records of 52 &#xD;
patients who were diagnosed with cancer cervix. The data &#xD;
abstracted from each subject’s medical record include the &#xD;
following: age, parity, clinical staging as per International &#xD;
Federation of Gynaecology and Obstetrics (FIGO), size of the &#xD;
lesion, complete blood count and length of the survival. Patients &#xD;
underwent surgery followed by appropriate chemo-radiotherapy &#xD;
or brachytherapy. They were followed-up after treatment every &#xD;
three months for the first two years and every six months for &#xD;
the next three years and annually thereafter. The five-year &#xD;
survival rate of these patients who were on complete follow&#xD;
up was analysed. The normal platelet count was considered as &#xD;
4.5×109/L. Data was entered into Microsoft excel data sheet &#xD;
and was analysed using Statistical Package for Social Sciences &#xD;
(SPSS) version 22.0.&#xD;
 Results: The mean age of the study population was 50.28 &#xD;
years. Among the 52 patients, 47 (90.03%) were with advanced &#xD;
stages of cancer cervix (stages IIB-IVB) and 5 (9.6%) belonged &#xD;
to early stages of cancer cervix (stages 1B-IIA). Forty patients &#xD;
(76.92%) had a platelet count less than 4.5×109/L, out of which &#xD;
2 patients were in early stage of cervical cancer and 16 patients &#xD;
were with tumour size less than 4 cm. A total of 12 patients &#xD;
had the platelet count more than 4.5 lac. All these 12 patients &#xD;
belonged to advanced stages of cancer cervix (stage IIB-IVB), &#xD;
p-value of 0.001. Among the 12, 11 patients had a tumour size &#xD;
of more than 4 cm, p-value 0.040. The mean five-year survival &#xD;
rate among patients with advanced stages of cancer cervix &#xD;
associated with thrombocytosis was 18%. &#xD;
Conclusion: The platelet count was found to have strong &#xD;
association with the tumour size, stage of the cervical cancer &#xD;
and five year survival rate thus making thrombocytosis to be a &#xD;
strong prognostic factor in cancer cervix.</description>

Abstract: Abstract &#xD;
Background: Intrauterine foetal demise or still birth refers to an antepartum or intrapartum foetal death &#xD;
occurring after 20 weeks of gestation. The majority of these deaths would probably be prevented with &#xD;
better access to expert healthcare and providing health education to the women. &#xD;
Aims: To find out the associated risk factors for antepartum intra uterine foetal demise and determine the &#xD;
probable cause of antepartum foetal demise &#xD;
Materials&amp; methods: This is a prospective study conducted at R.L JALAPPA Hospital attached to Sri &#xD;
Devaraj Urs Medical College, Tamaka, Kolar included all antepartum intrauterine foetal demises after 24 &#xD;
weeks of gestation between January 2021 to December 2022. &#xD;
Results: In the present study, the maternal causes of intrauterine foetal demise, hypertensive disorders &#xD;
contributed to 46.65% followed by anaemia. Amongst the foetal factors, foetal growth restriction seen in &#xD;
26.6%. Statistically significant difference was found between birth weight and gestational age, attributing &#xD;
to the foetal growth restriction. In placental causes abruption placenta accounted for 15.23% and idiopathic &#xD;
constituted 26.7%. Decreased perception of foetal movements was seen in majority of the cases. &#xD;
Conclusion: Finding the causes of IUFD will enable the development of a successful prevention strategy, &#xD;
preventing maternal problems in the process. Good antenatal care and detection of risk factors like &#xD;
preeclampsia, anaemia, FGR, congenital malformations are necessary to plan the next level of &#xD;
management. Not only that a potential life is lost, mothers experiencing intrauterine foetal demises suffer &#xD;
psycho-social consequences like anxiety, depression, stress disorders and stigmatization and also the future &#xD;
prospects of fertility and pregnancy outcome becomes one the major concerns for them, DC.</description>
